The submission of the Direct Deposit/ACH Authorization Agreement authorizes Rising Medical Solutions, LLC and its affiliates (“Company”) to electronically deposit payments through the Automated Clearing House (ACH) to the bank account provided for amounts payable to the accepted payee Party, under the Virginia Birth-Related Neurological Injury Compensation Program.

  1. This authorization Direct Deposit/ACH will remain in effect until written notification is submitted to Company to terminate the payment transactions.
  2. In the event that the account is changed or relocated to another bank or financial institution, Company may require up to 30 days advance notification of such change to redirect payments to the new account. Completion of a new Direct Deposit/ACH Authorization Agreement for the new account is required.
  3. All eligible disbursements will be made in accordance with Company’s standard payment terms as set forth in the agreement between Company and Virginia Birth-Related Neurological Compensation Program. Advance notice will be given to all payees if payment terms are altered or changed.
  4. Company reserves the right to initiate a reversing entry as permitted by the Rules of the National Automated Clearing House Association.
  5. Payment history also can be viewed online via the payee’s Rising VISIONTM portal account.
  6. Company has the right to change or terminate the Direct Deposit/ACH services for any reason upon advance notification to payees.
